If you need the Council to fund an adaptation to your home you must request an Occupational Therapist Assessment.
To request an Occupational Therapist assessment you will need to complete an online form that can be found on Cornwall Councils website, please see link below:

The Occupational Therapist will identify the assessed need and send through a referral with the adaptation recommendation (this starts the Disabled Facilities Grant application process).
If your adaptation application is for a child you will need to call 0300 1234 101 as it is not on the online service.
To be eligible to apply for the grant you must:
• Be disabled
• Intend to live in the property during the grant period (usually 5 years but this can be shorter, for example, if the person is terminally ill)
The assessment process helps establish whether you are able to receive a grant, which includes a financial assessment but also ensures that the adaptation being requested fits the grant criteria and that it is necessary and appropriate.
The outcome of the DFG assessment will be sent to both Cornwall Housing and yourself. It will mean that an adaptation will either be fully or part funded, or will not be eligible to receive funding, in which case an individual will have the choice to go ahead and pay for the adaptation themselves.
The outcome of the DFG assessment is not approval for the works to go ahead, the decision for works to go ahead will only be made once Cornwall Housing review your application against the guidance included in the Cornwall Council Accessible Housing policy.
DFG refusal
If you are unhappy with being refused a DFG there is no formal appeal against the grant assessment process as it is defined in legislation. However, you can raise a complaint to challenge the Council should they believe the application has been assessed incorrectly or the legislation or process has not been followed correctly.

The complaint will be dealt with by Cornwall County Council and will be completely independent of CHL.
